I agree to abide by the Association Rules and By-Laws as set down by the Queensland Police Service Rugby League Association (QPSRLA) and the Queensland Police Service (QPS) Code of Conduct. I will ensure that my behaviour complies with the expectations of the QPS, the QPSRLA and Community Standards. I am aware that my attendance at the championships may be cancelled by the QPSRLA for breaches of the rules, by-laws or poor behaviour. I declare that I am taking part in these State Championships at my own risk and that the QPS and QPSRLA will not be held liable for any injury or illness sustained by me during the course of this event. I am aware that the QPSRLA will take out limited insurance for this event and that, should I desire, I am personally responsible for obtaining personal insurance to cover any injury or illness I may sustain.
